Assistance is meant to offer meaningful support to individuals and families while they develop practical plans for meeting their own needs. The program requires clients to complete a service agreement or plan of action that addresses personal and professional development needs.
The Plan of Action
The Plan of Action/Service Agreement is an action plan based on the client assessment and re-assessment and must be completed with every case management client. A GCCSA Service Specialist and the client(s) work together to create an individualized Plan of Action, which supports a client’s movement toward self-sufficiency.
